Dear Parents and Guardians,
We look forward to starting a new school year at Good Hope Country Day School with you and your children! In this letter you will find a list of required medical documents for the start of the 2026-27 school year.
All students, from nursery through senior year, are required to have annual wellness exams with their primary care provider (PCP). The attached physical exam report, completed by your child’s doctor, must be submitted to the school no later than August 19, 2026. This form is also available through our enrollment portal, TADS. In accordance with USVI Law, USVI Department of Health and Human Services policy, and Good Hope Country Day School Policy, vaccine records and health exam reports are required of all students to start on the first day of school. If you have any questions regarding the requirements for school attendance please contact the School Nurse at [email protected]. Please plan to schedule a meeting with the nurse, using the provided email, prior to the start of school if your child has a chronic health condition and/or requires any medical accommodations during the school day. The nurse will be available on registration day from 12:00PM - 3:00PM for confidential health meetings.
Physical Exam/Wellness Exam Record: To be completed by your child’s physician no earlier than June 2026 and submitted prior to the first day of school.
- Due annually for all students in Nursery through 12th Grade
- If your student will be participating in Sports or After School Activities, your doctor will need to complete a preparticipation physical exam and submit the Medical Eligibility Form. No athlete will be eligible to participate in school sponsored sports until this form has been filed at school with clearance from their medical doctor. Our Athletic Director, Mr. Mondesir, will be verifying that these forms are on file before students are permitted to participate.
Immunization Record
- Official records must be on file on or before the first day of school. In accordance with USVI law, students will not be permitted to attend school until this requirement is met. Please see the Department of Health vaccine requirements notice.
- The following immunizations are required for school entry in the USVI: Hepatitis B, Diphtheria, Tetanus, Pertussis, Polio, Measles, Mumps, Rubella, Varicella, and Meningococcal. An annual flu vaccine is strongly encouraged as well as the primary series and boosters for Covid-19.
- Parents of 11 year olds: The first dose of the Meningococcal conjugate vaccine (MCV4) and a booster dose of Tdap are due within one month of eligibility. The HPV vaccine is optional but encouraged. Please see Vaccines for Preteens and Teens for more information.
- Parents of 16 year olds: The second dose of Meningococcal conjugate vaccine (MCV4) is due at this age.
- If your child has an exemption from vaccination, a Vaccine Exemption Form from the Department of Health must be submitted on or before the first day of school. You must bring a copy of your child’s physical exam report and a notarized certificate for exemption to the Maternal and Child Health (MCH) Division of the Department of Health to receive your authorization. You are encouraged to start this process early!
- Virgin Islands Immunization Registry (VIIRS): All students entering kindergarten or new students to the VI school system must complete the VIIRS form, which will then authorize immunizations to be uploaded into the immunization registry.
Parent Permissions Form:
- This includes permissions for: OTC medications selected by guardians, if needed communication between the nurse and PCP who signed the physical exam form, and for emergency medical treatment. These permissions are to be signed by parents/guardians. You will find these individually listed on page two (2) of the physical exam form.
If applicable:
- Prescription Medication Release Form: Signed by a parent and physician. A new form is required annually for prescription medications (including Oxygen) to be administered at school. Please schedule a time to meet with Nurse Cooper on registration day to discuss and drop off medications. Please understand, the nurse is unable to administer any prescribed medications, no matter the circumstance and meaning possible exclusion from school, without signed documentation from your child’s prescribing physician.
- Emergency Action Plans (EAP) from a Physician (For asthma, allergies, diabetes, etc.) The below listed action plans are simply suggestions, your doctor will decide which is best for your child.
