Health Documentation for Enrollment – GHCDS

Good afternoon Parents and Guardians,

As we prepare to welcome our students back to school, I am busy reviewing health records and preparing for how to best care for our student body throughout the year. As a reminder, an annual health exam is required for every student from nursery through senior year of high school. A copy of their annual Physical Exam Record and current Immunization Records (or Immunization Exemption Authorization from the Department of Health) are due by the first day to attend classes. Please review the Department of Health Immunization requirements for school attached below. It is time, if you haven't already, to make those appointments with your child's health care provider. You can email the completed forms or drop them off to the main office prior to the first day of school. Thank you to those of you who have already submitted!
 
You will find all required health forms attached to this email. Some forms may not apply to all students, for example the Medical Eligibility form for students who participate in sports and/or aftercare or the Emergency Action Plan for students with allergies. Please carefully read the "GHCDS Health Office Requirements" document to see what applies for your child. If you have any questions or concerns, please do not hesitate to reach out to me. 
 
Please note, if your child needs to take prescribed medications during the school day, their prescribing physician must complete a Medication Authorization form. I can not dispense any prescription medications without a signed form from your child's doctor. Please drop off medications with me on registration day with the appropriate documentation and in the original containers. 
 
I will be available on registration day for health record collection, medication drop offs, and parent meetings should you need or desire. If your child needs accommodations for their health and safety during the school year, please schedule a meeting with me for registration day to cover a plan of care to best meet those needs.
