Liability Waiver and Release Form

Dear Parent/Guardian,

We are thrilled your child will be joining us for ENCORE: A Summer Theater Experience at GHCDS! While we make every effort to ensure a safe, positive, and enriching environment for all campers, we kindly ask that you review and sign the following release form.

I, the undersigned parent or legal guardian give permission for my child to participate in the 2025 Summer Theater Camp hosted by Good Hope Country Day School (GHCDS). I understand that while every precaution will be taken to provide a safe environment, participation in this program involves physical movement, stage work, and the use of creative equipment and materials.

I hereby release and hold harmless GHCDS, its employees, volunteers, and agents from any and all liability, claims, or causes of action arising out of or related to any loss, injury, or damage that may occur while participating in camp activities, whether on or off school grounds. This includes but is not limited to activities related to acting, dancing, prop handling, and set construction.

I acknowledge that GHCDS staff will act in the best interest of my child and that I will be contacted in the event of an emergency. I authorize GHCDS staff to seek emergency medical treatment if necessary. I understand it is my responsibility to inform GHCDS of any relevant medical conditions, allergies, or special needs prior to the start of camp.

I give permission for GHCDS to photograph and/or record my child during camp activities and performances. I understand these may be used in promotional materials, including social media, newsletters, and future program marketing.
